Please ensure that your vaccine records are up to date before April 30, 2024 at 4:30 p.m.
If you are missing a vaccine:
  •  Book an appointment with Public Health at regionofwaterloo.ca/vaccines
  •  You do not need a valid Ontario Health Card to get a vaccine from Public Health.
  • Clinic appointments are available at 99 Regina St South, Waterloo and 30 Christopher Drive, Cambridge.

Visit the Vaccine Bus where vaccines will be provided (walk-in only, limited availability)

  • Tuesday, April 23 from 1pm – 4pm and 5pm – 7pm:  Wilmot Rec Complex (1291 Nafziger Rd, Baden)
  •  Wednesday, April 24 from 1pm – 4pm and 5pm – 7pm:  Ayr Curling Club (210 Northumberland St, Ayr)
  • Thursday, April 25 from 1pm – 4pm and 5 – 7 pm:  Woolwich Memorial Complex (24 Snyders Ave S, Elmira)

Book an appointment with your primary health care provider and upload your record after vaccination at regionofwaterloo.ca/vaccines.

  • Please note that some primary care health providers may not have the Meningococcal ACYW-135 vaccine (Menactra®, MenveoTM, Nimenrix®) available at their office.
No further action is required if:
• You received confirmation from Public Health that you will not be suspended.
• You already have an appointment booked with Public Health before May 1, 2024 – please keep your appointment.
If you have received any of the missing vaccines listed on the Suspension Order, report them online at regionofwaterloo.ca/vaccines.
If Public Health does not receive this information before 4:30 p.m. on April 30th, you will be suspended from school starting on Wednesday, May 1, 2024.
Please note that Public Health is unable to release health information to families or caregivers for students over the age of 16 years without their consent.

Appointments are available for any student who needs to catch up on their routine vaccines at Public Health clinics. Families can also contact their healthcare provider or a walk-in clinic for vaccine availability.

 Report your child’s vaccines to Public Health

It is the parent/legal guardian’s responsibility to report their child’s vaccines to Public Health if they received their vaccines from a health care provider.

All they need is a copy of their child’s vaccination record from their health care provider and their child’s health card number (if applicable). Watch this video for instructions on how to report.
