Waterloo Public Health – Immunization Records Update

A Message from the Region of Waterloo Public Health

In the coming weeks, Public Health will be sending out Suspension Orders to students over 16 years of age and to the parents/guardians of students under 16 years of age who continue to have incomplete vaccination records with Public Health.  If you receive a letter, you must update your (or your child’s) records with Public Health, or you/your child may be suspended from school.

Updated records must be received by Public Health before 4:30 p.m. on Monday April 6, 2026 to avoid suspension.  Suspensions for students with incomplete records start on Thursday April 9, 2026. 

Act now by:

  • Booking an appointment with your doctor to receive required vaccinations
  • Submitting up-to-date records online at Vaccine Reporting – Region of Waterloo

If you have questions about a letter you received, please visit www.regionofwaterloo.ca/vaccines where you can:

  • View a translatable version of the letter
  • Request or report vaccine records
  • Submit vaccine exemption forms
  • Book vaccine appointments
